For good and valuable consideration, including the opportunity to attend or participate in certain events organized or sponsored by SHOOK Research (collectively, “Events”), the receipt and sufficiency of which is acknowledged, I hereby understand, acknowledge and agree to the following:

SHOOK Research reserves the right to photograph, film/videotape me and record my voice at any and all Events (collectively, “Recordings”), and to use the Recordings and any material I provide and my name, voice, likeness, image and information about me (collectively, the “Appearance”), and I hereby grant SHOOK Research the permission to do so.

SHOOK Research may use, without requiring my express permission at each instance, the Appearance in publications or other media material produced, used, or contracted by SHOOK Research including but not limited to: brochures, invitations, newspapers, magazines, presentations, websites, advertising, publicity, public displays, and exhibitions, etc.

I hereby freely and without restraint consent to and give unto SHOOK Research and its subcontractors, agents, successors and assigns or anyone authorized by SHOOK Research (collectively referred to herein as the "Releasees") the unrestrained right in perpetuity to own, utilize, or alter the Recordings, in any manner the Releasees may see fit and for any purpose whatsoever, all of the foregoing to be without limitation of any kind. I hereby stipulate that the Recordings are the property of SHOOK Research to do with as it will, in perpetuity and in all languages, for all now known and/or hereafter existing uses, media, and forms, including, without limitation, all copyrights and related rights.

Releasees may utilize the Recordings or not as they choose in their sole discretion without affecting the validity of this Agreement. I hereby waive the benefits of any provision of law known as “droit moral” and/or “moral rights” or any similar law in any jurisdiction. It is understood and agreed that SHOOK Research shall have the right to combine the Appearance with any other materials of SHOOK Research and SHOOK Research shall have the right to edit, rearrange, vary, change, alter, modify, and to, and delete any portion of the Appearance.

I hereby waive to the fullest extent that I may lawfully do so, any causes of action in law or equity I may have or may hereafter acquire against the Releasees or any of them for libel, slander, invasion of privacy, copyright or trademark violation, right of publicity, or false light arising out of or in connection with the utilization by the Releasees or another of the Appearance or Recordings.

By registering for this event, you acknowledge and agree that your name, firm affiliation, contact information and other information provided may be shared with event sponsors and select third-party partners for purposes related to the event, including but not limited to networking, follow-up communications, and promotional offers and marketing communications. You may opt out of such communications at any time by contacting SHOOK Research directly.

I hereby irrevocably release SHOOK Research (including its licensees and assigns) from any claims of any nature (including rights of privacy and publicity) with respect to the use of the Appearance, Recordings, and/or Participant Information and/or the rights granted herein. "Participant Information" refers to any personal or professional information provided by me in connection with my participation in the event, including but not limited to my name, firm affiliation, job title, email address, phone number, and other information provided. I will indemnify and hold harmless SHOOK Research, its successors, assigns, affiliates, licensees, sub-distributors, agents, officers, directors, employees, and shareholders from and against all costs, damages, and claims arising from any and all claims involving the use of the Appearance, Recordings, and/or Participant Information, and/or any breach or alleged breach by me of any representation, warranty, or agreement hereunder.

I understand that if I do not wish to have my Appearance recorded for distribution, I will make notify, during the applicable Event, the photographer and/or the Event organizers, and/or notify, in advance of the applicable Event, SHOOK Research via e-mail at contact@shookresearch.com, failing either of which, I will be deemed to have consented to the Recording.

I understand that I may not use, copy, distribute, download, share, alter or modify any Recordings without the advance written permission of an authorized designee from SHOOK Research.

I hereby certify that I am over the age of eighteen, and that I have read, understood, and agreed to the foregoing; that I have had the chance to have my own legal counsel advise me about this document and that I have either done so or freely decided not to do so; and that other than as specified herein, SHOOK Research has not said or promised anything to induce me to grant the permission and release herein.
